If a member intends to terminate their Coop membership, they can forward to forms@ampc.org.ph the scanned copy of their accomplished Membership Termination Form and 1 valid ID.
Our Membership Termination Form is downloadable through our Coop website, www.ayalacoop.com
• I would like to use my share capital as my loan payment. How do I proceed?
Please take note the using your share capital will result in termination of your membership.

We regret to inform you that we do not allow partial withdrawal as it will result to Termination of the membership.
